Section 4 - Documentation

Please submit a full, eligible, current a valid copy of each of the following documents:

  1. Articles of Incorporation or Bylaws (name and address must be shown)
  2. Document showing company registration number with national authorities
  3. Shares Certificate for the shareholders included in Section 2
  4. Resolution or appointment of Directors or Management officers included in Section 2

Please submit at least one of the following documents as evidence for the company’s source of funds and business activity:

  1. Last three (3) Bank Statements
  2. Tax Returns / Tax Declaration
  3. Letter signed by certified lawyer or accountant detailing the origin of your funds, AND/OR

As proof of the company’s shareholders and director’s identity, please submit a full, eligible, current and valid copy of one of the following documents for each one of them:

  1. Passport ID Page
  2. National Identify Card, OR
  3. Driver License

As proof of the company’s shareholders and director’s address, please submit a full, eligible and translated (if required) copy of one of the following documents for each one of them:

  1. Recent Utility Bill issued within the last three (3) months, other than credit card statements or mobile phone bill
  2. Recent Bank Statements issued within the last three (3) months, OR
  3. Driver License Address Information (only if not used as proof of identify)
